Optimizing Blood Glucose Monitoring for the Frail Elderly in Long-Term Care Settings

Canada’s population is aging, and the prevalence of diabetes and other chronic diseases is on the rise. These factors pose significant implications for the publicly funded health systems of Canada, as policy-makers and clinicians alike consider how to provide the best care possible for the elderly within the context of limited public resources. A growing body of scientific evidence indicates that one area of diabetes management — the monitoring of blood glucose in individuals with diabetes — is often overused in the general population. Following on this, questions have been raised regarding vulnerable populations of individuals with diabetes, including elderly residents living in long-term care facilities. This educational and interactive webinar is geared to clinicians, long-term care administrators, and others involved in the support of this patient population. Ms. Kelli O’Brien and Ms. Heather Brown will introduce and moderate this webinar. Dr. Ann Colbourne  will present the current scientific evidence on blood glucose monitoring in diabetes, Dr. Kenneth Rockwood will focus on the topic of frailty, and participants will have an opportunity to pose questions and test their knowledge through a pre- and post-quiz.
